Dr Chris O'Connor is a critical care physician and the Director of Medical Informatics at Trillium Health Centre in Mississauga. Dr O'Connor lead the development of Trillium Health Centre's order set project which received the first Ministry of Health and Long Term Care award for Innovation in Patient Safety and Quality. In 2006 Dr. O'Connor established Open Source Order Sets. In 2007 Dr. O'Connor received the COACH, Canada's Health Informatics Association, Emerging Leader in Health Informatics Award.
Katharine is a registered nurse with 24 years of nursing experience. She completed her MN/ACNP at the University of Toronto. She has worked in a variety of different practice settings including, critical care, emergency, and cardiac surgery. In 2004 Katharine was a clinical informatics specialist with the Trillium's THINK initiative. She completed a certificate in Healthcare Information Technology at the University of Connecticut in February 2006. Katharine was the lead of the order set project at Trillium Health Centre and co-chaired the order set committee. Katharine currently leads Open Source Order Sets full time and works closely with all the OSOS partnering hospitals across Canada.
Anne Marie is a Registered Nurse with extensive clinical experience in a variety of practice settings including medicine, palliative care, orthopedics, neuro trauma and general surgery. She currently practices at Soldiers Memorial Hospital in Orillia in Hemodialysis. Anne Marie works full time with Open Source Order Sets as an order set consultant and is skilled in order set formatting, development, and order set lifecycle methodology. Anne Marie provides support and assistance to all OSOS partners order set projects.
Myrna Eddison attained her RN and midwifery diploma from Belfast, N Ireland and London, England and returned to her country of birth to work in neurosciences. After 12 years she transferred to neonatal nursing and found her passion. After years as staff nurse and 8 years as clinical leader of the intensive care nursery, she transitioned to a position of clinical informatics specialist with the THINK initiative and then to the order set group as consultant for the Women's, Children's and Neurosciences Services at Trillium Health Centre. Myrna has 41 years of experience. She has retired, but continues as neonatal educator and volunteer in the obstetric/neonatal department on a casual basis and enjoys her second passion developing and formatting order sets with OSOS.
Hazelynn Kinney is a registered nurse with a masters degree in nursing from the University of Toronto. She has 12 years of nursing experience ranging from rehabilitation, coronary care, cardiac intensive care and neurology. Hazelynn was a clinical nurse specialist for the Stroke Network. Currently, she is the clinical educator for the coronary care unit at Trillium Health Centre. Prior to joining Open Source Order Sets Hazelynn was involved with the Best Practice Guideline Initiative for Long-Term Care and participated on the developmental panel for the Registered Nurses Association of Ontario's Best Practice Guideline for Hypertension. Since then her focus has been promoting and standardizing best practices in various health sectors. With this experience Hazelynn assists OSOS by developing and formatting order sets and clinical protocols with the goal of improving the quality and safety of care provided to clients across the OSOS network.
Jiten Jani is a registered pharmacist with 6 years of hospital pharmacy experience. Following completion of his Bachelors in Pharmacy from the University of Toronto, he went on to complete his residency at Mount Sinai Hospital. Jiten then specialized in Nephrology at Credit Valley Hospital working in the inpatient and outpatient settings providing pharmaceutical care to Hemodialysis, Peritoneal Dialysis, Transplant, and CKD patients at the Kidney Care Clinic. He was further involved in cardiac teaching for the cardiovascular rehabilitation program and was the lead pharmacy analyst for the implementation of hospital-wide eHealth solutions as part of the EDDIE project. Currently Jiten is working at Cancer Care Ontario, assisting with the implementation of CPOE solutions across Ontario and pursuing his PMI Project Management Professional certification.
Shelley is a registered nurse with 25 years of nursing experience. She completed her MN at Athabasca University. She has worked in a variety of different practice settings including, PACU, emergency, general surgery and education. Shelley was the clinical lead of the order set project at Vancouver Island Health Authority and co-chaired the order set committee during its initial phase. Shelley currently works as a Medical Clinical Leader at VIHA and is a consultant for Open Source Order Sets part time.
